Commander Portal
MODS is now sending emails to Commanders notifying them of unviewed profile conditions. To view all profile conditions, ensure the IMR Exclusion button at the top of Commanders Portal is selected. If you should not be receiving these emails, please contact your supervisor or the MODS help desk to remove your access.

A USAR UIC reorg/realignment took effect today, 1 October 2025. Medical Readiness systems such as MODS will be impacted. MODS uses Army Org Server (AOS) as it’s source for UIC data, so unit alignments should adjust in the system within 24 hours of AOS’ implementation. Please verify your UIC alignments in AOS. Note - CPAM, CPAA, and other admin roles may not align properly after the reorg and may need to be manually aligned. Please contact your next higher approval authority for any necessary property role alignments. Please contact the MODS helpdesk if you experience alignment issues after 3 October 2025, primarily if the UIC in MODS does not match what your team updated/validated in AOS.
MHA (PHA/DHA/Referral Tracking)
OTSG/MEDCOM G-37 Medical Readiness Division and the Medical Operational Data System (MODS) streamlined the health assessment forms eliminating questionnaire duplication. The SM completing a deployment form during a PHA doesn’t have to answer duplicate questions while having the opportunity to review responses before submitting. The Provider portions aligned with both the assessments populate the completed form for verification prior to submission. There is a message above each question showing where the response originated. Note the health assessment forms must be completed within 30 days with exception of behavioral health questions, those are 7 days. If forms aren’t duplicating responses please contact MODS helpdesk at usarmy.ncr.hqda-otsg.mbx.mods-helpdesk@army.mil, with the survey form and question.
MODS Control Panel
MODS is making a number of updates to User Management (MCP) starting in October through the end of the year. These changes include sending access request notifications to approvers. In the meantime, if you need assistance with an access request, please refer to the POC Map on the readiness applications (MWDE, MEDPROS, Portal, and MHA) or contact the MODS Help Desk. The MODS Support Team will continue to post messages to the applications as updates are made.
